This type of reference document is a specific form/questionnaire created by a program that can be requested as part of their application requirements. These forms are available within the program descriptions of the program's that are requiring them and it is the student’s responsibility to provide the link to the form within their reference request.
In addition, you must create a reference request for them the same way you would request a reference letter in CaRMS Online. The difference is that you can select "Applicant Support Form" as the "Reference Document" when completing the request. You will need to give your referee the form so they can submit it accordingly.
