What is the FM/ES reference form and how can it be submitted by my referees?

For the FM/ES match, a form is required to be filled by your referee.

To help applicants and referees in this process, CaRMS has incorporated this form online on the referee’s platform. When you request a reference from your CaRMS online account, the blank form will automatically be generated in your referees account. Then, the referee will simply have to fill out boxes and answer each question on the form. The form also includes a section where your referee can include a letter of recommendation.

Once completed, they simply submit it. This form does not need any signature from your referee.
