How do I submit a reference assessment form for the FM/EM match?

Assessment forms for the FM/EM match are built into the CaRMS Online system and can therefore only be submitted electronically through your CaRMS Online referee account.

Once the applicant has sent the request, it will automatically appear on your reference request list. You then need to fill it out electronically. You are not required to upload any documents, you will simply fill out the boxes and answer each question on the form. The form also includes a section to add a letter of recommendation.

Once you have saved the form, you can preview the letter by clicking on "FM/EM referee assessment form" in the “type of request” column. 

Please note that to save the form you must fill out every section. Please ensure to click “submit” once completed.
