What is a medical school transcript?

The medical school transcript should reflect the total, complete academic history at the institution. All courses should be recorded on the transcript whether attempted and/or completed. The courses should be listed in the academic period(s) in which they were attempted and/or completed.

There are two options for adding this document to your account:

1. Upload the document yourself in the Document Tracking section of your CaRMS Online account
2. Ask your medical school to upload your document using the CaRMS Online Undergraduate Portal

Note: all uploaded transcripts must be in PDF format and no larger than 5 MB.
