I completed the CaRMS self-identification questionnaire. Can I have a copy?

Once you have completed and saved the CaRMS Self-Identification questionnaire, a PDF copy will be available within your DOCUMENT TRACKING section of your CaRMS Online account.

1. In your CaRMS Online account, under MY DOCUMENTS, click on DOCUMENT TRACKING.

2. In your list of documents, search for CaRMS Self identification questionnaire and click on the hyperlinked title to download a PDF copy locally.
