This document can only ever be accessed and viewed by the PGME offices of Québec faculties to ensure the requirement has been met. 

File Reviewers, Program administrators, and Program Directors are not able to view or access your proof of COVID-19 vaccination status/Declaration of medical exemption, even if it is submitted as part of your application.
