I am completing an application in both English and French, do I need to complete the self-identification questionnaire for each language?

Yes, you will need to have the questionnaire in both languages. Your application language will need to be set as bilingual, and your CaRMS self-identification questionnaire will automatically be created in both English and French once saved.
