How will my questionnaire information be shared with programs?

The information within your CaRMS Self-Identification Questionnaire is not automatically provided to programs as part of your application package. If you complete the questionnaire and wish to share it with programs that require it, you must assign it to them along with your other application documents once program application opens.

Once File Review has begun, your questionnaire is shared with the program if you have assigned it as part of your application.

However, no information regarding sexual orientation or gender identity is shared with programs in the questionnaire. This information is collected solely for data purposes.
